Our Mission The Mercy Inpatient Rehabilitation Center program at St. Joseph Mercy Port Huron is committed to restoring the persons we serve to meaningful lifestyles by providing select, innovative and efficient services in a coordinated continuum of care.
The comprehensive, integrated Inpatient Rehabilitation program meets our mission and goals by providing quality services that address the patient’s impairment, activity and participation.
Our Program Inpatient Rehabilitation services are provided on a 10-bed unit. The rehabilitation staff has dedicated themselves to providing comprehensive, state of the art services that help patients attain their highest level of functioning. The standards for each plan are based on the patient’s goal for independence. When needed, we also provide home evaluations in preparation for discharge.
Who is Admitted into Our Care The following are some examples of rehabilitation diagnoses that may be appropriate for admission to the program:
- Stroke (CVA)
- Brain Injury
- Amputation
- Spinal Cord Injury
- Major Multiple Trauma
- Hip Fracture
- Multiple Sclerosis
- Parkinson’s Diseae
- Guillain Barré
- Other progressive neurological impairments
Admission Criteria Inpatient medical services are provided for patients 18 years of age and older. Admission criteria includes:
- Medical stability
- Motivation and ability to participate in therapies at least five days per week; a minimum of three hours per day
- An identified discharge plan
- A 24-hour per day need for rehabilitation nursing and medical care
- Potential for functional improvement
- No recent completion of an inpatient rehabilitation program for the same injury or illness, unless a change of condition has occurred and is documented
- Require Occupational & Physical Therapy treatment
Your Care During your stay in the Mercy Inpatient Rehabilitation program, you will be involved in a daily program that will include activities designed to help you meet your rehabilitation goals. As an integral member of the team, you will be a part of establishing those goals.
Your team is lead by a Physiatrist - a medical doctor who specializes in rehabilitation. The Physiatrist and the team will work with you to determine a plan of care to achieve your goals based on your unique needs. Other members of the team include:
- Rehabilitation nurses
- Occupational therapists
- Physical therapists
- Speech/Language pathologists
- Social worker
- Case manager
- Nutrition counselor/registered dietitian
