St. Joseph Mercy Oakland Receives $20,000 CVS Caremark Charitable Trust Grant

Monies to Fund Hospital’s Community Health Ambassadors Program

Pontiac, Mich.—St. Joseph Mercy Oakland (SJMO) has received a grant of $20,000 from the CVS Caremark Charitable Trust, the private foundation created by CVS Caremark Corp.  The purpose of the foundation is to help people on their path to better health by providing funding for initiatives that support access to health care and children’s programming.  The grant to SJMO is part of $2.6 million in grants awarded to 66 non-profit organizations across the country as part of the 2012 CVS Caremark Charitable Trust grant cycle.

Monies received from the grant will benefit SJMO’s Community Health Ambassadors (CHA) program, which promotes healthy living, screens for and prevents chronic disease and engages citizens to be active in their health care.

The goal of the program is to extend SJMO’s community outreach efforts in the greater-Pontiac area.  Working with the hospital’s Community Programs Department, the CHAs will build new and strengthen existing relationships within the community, such as churches, schools, community centers, grocery/convenience stores, salons/barbershops and block clubs.

“We are honored to receive this grant from CVS Caremark for our Community Health Ambassadors program,” said Jack Weiner, SJMO President and CEO.  “It recognizes our commitment to help improve the health of people in the community who otherwise may not have access to health care because of economics, transportation or other issues.  The grant will allow us to continue our outreach to people right in their neighborhoods.”

The CHAs arrange outreach and health education opportunities, which include screenings for blood pressure, blood sugar and cholesterol. They are responsible for the planning and implementation of the events, coordinating them with the SJMO Community Programs Department, conducting the screenings and promoting the department’s other events and classes.

The CHAs come from a variety of professional backgrounds:  nursing accounting, home health, community activism, entrepreneurship and social services.  They serve the city of Pontiac, but as other pockets of need in northern Oakland County are identified, current or future CHAs will be assigned to those areas.  The CHAs are well-connected in their communities and dedicated to health promotion and disease prevention.

CVS Caremark Charitable Trust grants were awarded to organizations aligned with CVS Caremark All Kids Can, a signature program of the trust and CVS Caremark, which is committed to helping children of all abilities to be the best that they can be.  These organizations increase access to specialized medical and rehabilitation services and provide inclusive opportunities for physical activity, play and social enrichment.

CVS Caremark Charitable Trust grants also were awarded in support of innovative approaches that increase access to health care for underserved populations, including mobile health innovations that bring health care services to locations where people need them most. In addition to the grants awarded by the CVS Caremark Charitable Trust, CVS Caremark provided more than $3 million in 2012 to support a variety of programs focused on making a positive impact in local communities.
