Listed are the forms that need to be completed by students BEFORE they begin clinical rotations within the Saint Joseph Mercy Health System.

Please submit all forms and immunization information, at the same time, to the Affiliating Schools Coordinator listed below, BEFORE you start your clinical rotation. Please be sure your paperwork is labeled with your name, class and unit.
